Lions International District 18-B

Instructions for use:

These forms are for the exclusive use of the Lions Clubs located in District 18-B only. Only the club secretary or president should file reports for the club. To enter the monthly report click on the link for your zone. If you use the wrong form, the information you submit will be delivered to the wrong people. Be sure to fill out the entire form and check it carefully before pressing the submit button. When you submit your report it will be sent by e-mail to the following District Officers: District Governor, Vice District Governor, Zone Chairman, and District Newsletter Editor (In some cases you will be asked to hard copy to one or more officers who do not have e-mail). If you notice a mistake after you submit the form, press the back key and correct the mistake. Please note in the comments that this is a corrected copy. Then resubmit the report.

After you submit your report, a confirmation page will appear with your report as sent. You can print this page for your records or you may save the page to your computer by selecting "File" then "Save As". This configuration will not allow you to e-mail this page correctly. To e-mail your report, you will need to save it to your computer then attach it to your e-mail.

Once you close your viewer it will not be possible to reopen your report. Be sure to save or print your report.
