Eligibility:

Every Lion/Lioness who completes the following requirements between May 1, 20__ and April 30, 20__ may win the Golden Chain Award each year he/she meets the qualifications.

REQUIREMENTS:

The Order of the Golden Chain contains two equally important parts. Part I includes five activities which are mandatory. Part II lists twenty-four (24) activities in which a Lion/Lioness may participate. To qualify, a Lion/Lioness must complete all five mandatory activities in Part I and complete eleven (11) of the twenty-four (24) options in Part II.

Completion of Part I and Part Il before April 30, 20__ qualifies the Lion/Lioness for the Award. The completed form must be mailed to the District Chairman of the Golden Chain Award  postmark deadline is April 30, 20__.


PART I

THE FOLLOWING ACTIVITIES ARE MANDATORY:

 _____  A.     A Lion/Lioness must attend a minimum of 75% of his regular club meetings during the period between May 1, 20__ and April 30, 20__. Missed Meetings may be made up by attendance at a local Board of Directors Meeting or at Meetings of other Lions/Lioness Clubs.

 ____  B.     Sponsor a new member in a Lions/Lioness Club and help him or her become an active member.

 ____  C.     Attend a local Board of Directors Meeting.

 ____  D.     Be active as a local Committee Member.

 ____  E.     Attend a Zone or Cabinet Meeting.

PART II

OPTIONS

_____ 1. Serve as active local Chairman of internal or external Committee.

____ 2. Submit a complete plan for a new project to the local Board of Directors.

____ 3. Serve as a local Officer or Director.

____ 4. Report an acceptable article for publication covering an event or project of your club. Submit to local TV, Radio, Newsletter, or District Bulletin.

____ 5. Attend the Georgia Lions State Convention.

____ 6. Attend the International Lions Convention.

____ 7. Join the 10 Million Dollar Club — Georgia Lions Lighthouse Foundation.

____ 8. Join the Century Club — Georgia Lions Camp for the Blind.

____ 9. Attend the USA/Canada Forum.

____ 10. Attend the District Rally.

____ 11. Attend a Zone or Cabinet Meeting, in a addition to the mandatory activity.

____ 12. Serve as a member of a successful Extension Team and make (3) visitations within the qualification period. qualification period.

____ 13. Attend a work weekend at the Georgia Lions Camp for the Blind.

____ 14. Work White Cane Day.

____ 15. Serve as District or State Chairman of a committee. Office held: _______________________________________
